=== Peripheral edema and hypoalbuminemia === Kwashiorkor is a form of protein deficiency, which can result in both osmotic imbalances and irregularities in the lymphatic system. Kwashiorkor is most notable for peripheral edema. The presence of edema in kwashiorkor is correlated with very low albumin concentration (hypoalbuminemia). Edema results from a loss of fluid balance between the hydrostatic and oncotic pressures across the capillary blood vessel walls due to the lack of protein which affects the body's ability to draw fluid from the tissues into the bloodstream. Low albumin concentration influences negatively the strength of oncotic pressure. Failure leads to fluid buildup in the abdomen, resulting in edema and belly distension. Furthermore, the release of antidiuretic hormone is stimulated by hypovolemia, also leading to the development of peripheral edema. Plasma renin is also stimulated, promoting sodium retention. It is important to distinguish the pathophysiology of marasmus and kwashiorkor when it comes to treating malnourished children who may have hypovolemic shock that is caused by an acute loss of salt and water. === North Africa === The Medical Research Council decided that the time had come for field trials of penicillin. The location of centres to receive the drug was kept secret so as to not provoke demand for the drug when it was still in short supply. Florey was asked to go to North Africa, where the North African campaign was ongoing. He travelled to Algiers on the hospital ship HMHS Newfoundland in May 1943. On 29 June he was joined by Hugh Cairns, another Rhodes Scholar from Adelaide, who now held the rank of brigadier in the British Army, and was in charge of St Hugh's Military Hospital (Head Injuries) in Oxford, who brought with him a stockpile of 40 million Oxford units of penicillin. Florey resisted well-intentioned efforts by the War Office to grant him military rank. Over the next two months Florey and Cairns flew back and forth between Algiers, Sousse and Tripoli, with a week in Cairo. They treated over one hundred cases and compiled a report that ran to over one hundred pages. He gave lectures on penicillin, and his report contained recommendations for training of medical officers in its use. The fighting in North Africa ended in May 1943, so most of the cases Florey saw were not recently wounded soldiers, but ones with old wounds that had not healed; battle casualties began arriving again after the Allied invasion of Sicily in July. He considered that the source of infection in many cases was from the hospital rather than the battlefield, and advocated changes to the way that patients were treated to take advantage of the properties of penicillin.

=== Antifungal activity === Lactoferrin and lactoferricin inhibit in vitro growth of Trichophyton mentagrophytes, which are responsible for several skin diseases such as ringworm. Lactoferrin also acts against the Candida albicans – a diploid fungus (a form of yeast) that causes opportunistic oral and genital infections in humans. Fluconazole has long been used against Candida albicans, which resulted in emergence of strains resistant to this drug. However, a combination of lactoferrin with fluconazole can act against fluconazole-resistant strains of Candida albicans as well as other types of Candida: C. glabrata, C. krusei, C. parapsilosis and C. tropicalis. Antifungal activity is observed for sequential incubation of Candida with lactoferrin and then with fluconazole, but not vice versa. The antifungal activity of lactoferricin exceeds that of lactoferrin. In particular, synthetic peptide 1–11 lactoferricin shows much greater activity against Candida albicans than native lactoferricin. Administration of lactoferrin through drinking water to mice with weakened immune systems and symptoms of aphthous ulcer reduced the number of Candida albicans strains in the mouth and the size of the damaged areas in the tongue. === Toxicity === Today nicotine is less commonly used in agricultural insecticides, which was a main source of poisoning. More recent cases of poisoning typically appear to be in the form of Green Tobacco Sickness (GTS), accidental ingestion of tobacco or tobacco products, or ingestion of nicotine-containing plants.
